All manuscripts must be submitted through ScholarOne.
Manuscript
Each reference should be cited in the text. In the reference list, number the references according to the order in which they are first cited in the text and format them in Vancouver style.
Upon manuscript acceptance, you must submit the following:
- Licence to publish [PDF], signed by each author
- Licence to publish fillable [electronically fillable PDF], signed by each author
- ICMJE Form for Disclosure of Potential Conflicts of Interest, signed by each author
Where applicable, you may also need to submit:
Permission for acknowledgment [PDF]
If you acknowledge in your manuscript anyone for a contribution that goes beyond administrative assistance, you must obtain written permission from that person to publish his or her name.
Permission for personal communication [PDF]
If you wish to cite a personal communication, you must obtain written permission from that person to publish his or her name. Include the person's affiliation, and the year in which the information was communicated.
Patient consent
If your article contains a case description of an individual patient, you must confirm on submission that you have obtained from the patient fully informed, voluntary and written consent to publish. We recommend that you use our form, or something similar. We do not require that you submit a copy of the documentation but we may ask to see it. If the patient is a minor or incapable of providing informed consent, you must have obtained consent from the legal guardian or power of attorney/proxy/substitute decision-maker. If the patient is deceased, then you must have obtained consent from the executor(s) or estate administrator(s).
Written informed consent from the patient(s) to publish in the journal must include that you have:
- advised the patient(s) that their name and initials are not included in the manuscript and that you have endeavoured to conceal their identity, but that anonymity cannot be guaranteed and someone may recognize them, especially if there are images with distinctive body markings
- advised the patient(s) that materials published in CMAJ may be reproduced in other works, in print or digital format; this may include electronic communication such as social media, also works that are not derived from CMAJ but for which CMA will grant a license to publish or re-use in other formats, including online publications and medical books (and those in languages other than English or French)
- advised the patient(s) that once the manuscript has been accepted for publication they cannot revoke consent

Copyrighted materials
Permission to reproduce material
If your article contains a previously published table or figure, you must obtain written permission from the copyright holder (usually the publisher) to reproduce the material in CMAJ's print and online editions. In most cases, you must do this even if the article, table or figure is your own work. Please also forward a copy of the tables or figures from the book, journal or online source in which they were first published.
